Reporting


A work-related injury or illness is one that:
  • Occurs at work.
  • Requires medical treatment.
  • May or may not result in time off work.
The WCB requires information from health care providers treating an injured or ill worker to determine their entitlement to benefits and services.

Initial Medical Visit

All work-related injuries and illnesses must be reported to the WCB. If you are a doctor, please complete a Physician's Report (Form 8) following your initial assessment, and submit it to the WCB regardless of whether or not the injury or illness results in time off work. When completing the form, please mark the New Injury box.

Follow-Up Visits

Follow-up medical visits should also be documented on the Physician's Report (Form 8). When completing this form for a follow-up visit, please mark the Follow-Up box.

Physiotherapy and Chiropractic Treatments

Physiotherapy and chiropractic treatment should be documented on the corresponding Physiotherapy Report and Chiropractor Report and may be submitted electronically to the WCB through Online Services. Reports should be completed at the initial assessment as well as for progress and discharge.

Assessments

If a functional assessment is warranted, the findings should be documented on the Fitness for Work Assessment or Functional Scan form.

When completing paper forms, please type or use a ball point pen and press firmly. Forms must be complete and legible.
